We are seeking a dedicated Registered Nurse Care/Case Manager to join our healthcare CONSULTING team. This is a professional PART TIME employee position for Virginia licensed RNs only. We will also consider/accept LCSW, PTs, OTs and SLPs. Caring Considerations is a well-respected, established Aging Life Care™ Management (aka geriatric care/case management) company. This is NOT home care.
HOURS: This FLEXIBLE part-time position is NOT conducive to having another job. The position offers excellent scheduling flexibility. Client billable client hours range from 10-25 a week and can vary. Client load ranges between 5-15. Work is during business hours, but client emergencies require being available 24/7 occasionally. Your seasoned collaborative Team support is ALWAYS available for the infrequent client emergency response. A Northern Virginia residence (in your home office) is mandatory. Sorry, this is not a remote position.
EDUCATION/SKILLS:
Virginia Licensed Registered Nurse (RN, PT, OT, SLP) in Virginia with a minimum of two years’ experience working with older adults in a clinical or home-based environment. Home Health Care experience is very valuable.
_ TYPICAL DAY: Client Dr. appointment advocacy; scheduling, medication oversight; consulting about home safety; care coordination/options during a transition from Hospital to Rehab to Home; family caregiver coaching, ongoing assessment of client needs; suggesting resources such as daily money management, home health care, hospice, etc. etc._
REQUIRED SKILLS INCLUDE:
· Compassion and empathy for older adults and their families.
· Up-to-date technology knowledge is a must.
· Ability to honor client choices. Providing guidance.
· Excellent communication skills with various professionals, ages, ethnic and socioeconomic backgrounds via phone, email (writing) & personal contact
· Sense of urgency in a crisis. Responsive, Prompt, Reliable
· A desire to work in a mostly autonomous, consultative position that leads to improving the quality of life of others.
· Develop care plans
· Medical advocacy. Dr. visits. Medication Management
· Knowledge of dementia
· Family caregiver coaching
· Ability to lift transportation DME (
· Provide client transportation on a limited basis with manager approval
· Ability to research, solve problems, juggle and coordinate tasks.
· Some knowledge of community-based services and resources for older adults through private and public entities is very helpful
· Ability to discern appropriate care manager roles and responsibilities vs. other disciplines. Understand boundaries.
· Fiduciary responsibility and respect for clients and their families
· Participate in a care management team including weekly client video review meetings
· WORKING ENVIRONMENT: Your Home Office and client homes, senior/assisted living, hospitals, SNFs.
· NO hands-on personal, medical care is provided to clients.
· Background check required
· On-call approximately 7 weekends/holidays per year.
