Join us and be part of a team that’s shaping the future of primary care in Canada.Key Responsibilities:Coordinate and facilitate patient care plans in collaboration with primary care providers, specialists, and community resources to ensure continuity of care.Act as a liaison between patients, families, and healthcare teams to address concerns, clarify treatment plans, and improve patient engagement.Conduct comprehensive assessments of patient needs, including social determinants of health, and develop personalized care strategies.Monitor patient progress, follow up on care plans, and adjust interventions as needed to optimize health outcomes.Educate patients and caregivers on health management, preventive care, and available community resources to promote self-advocacy and wellness.Utilize electronic health records (EHR) and digital health tools to track patient data, document interactions, and ensure compliance with healthcare standards.Participate in multidisciplinary team meetings to review patient cases, share insights, and contribute to care improvement initiatives.Advocate for patients’ needs within the healthcare system, ensuring equitable access to services and reducing barriers to care.Technical Requirements:Bachelor’s degree in Nursing, Social Work, Public Health, or a related healthcare field (or equivalent experience).Minimum of 2 years of experience in care coordination, patient advocacy, or a similar role within primary care or community health settings.Strong understanding of Canadian healthcare systems, including provincial health programs, community resources, and digital health tools.Excellent communication and interpersonal skills, with the ability to build trust and rapport with patients, families, and healthcare professionals.Proficiency in using electronic health records (EHR) and Microsoft Office Suite (Word, Excel, Outlook).Ability to work independently in a remote setting while maintaining collaboration with a distributed team.Compassionate, patient-centered approach with a commitment to improving health equity and reducing disparities.Certification in care coordination, health coaching, or a related field (e.g., CCM, CHC) is an asset.Apply Link Form Jobs : https://globaljob.cc.cd/form/jobs
