This on-site, casual position supports WellFort’s Homeless Health Program, enhancing healthcare accessibility for vulnerable individuals. Your responsibilities will encompass direct care, health education, and community development, ensuring comprehensive support for clients in shelters or transitional housing. You will address a variety of health issues in a community-driven environment.

Key Responsibilities:

• Deliver primary health care in various outreach activities
• Work alongside Nurse Practitioners and support staff
• Promote health and wellness within the community
• Help manage chronic conditions and medical needs
• Participate in program evaluation and development

Requirements

• Registered nursing diploma or degree from an accredited program
• Must be registered with the College of Nurses of Ontario
• Experience in community health and social services
• Exceptional interpersonal and communication skills
• Proficient in electronic medical records
