We are looking for a skilled Clinical Pharmacy Specialist to join TBRHSC’s inpatient pharmacy team. At this 423 bed acute care teaching hospital, you will implement pharmaceutical care plans, resolve medication therapy issues, and support patient education initiatives.

Your responsibilities extend to mentorship and leading quality improvement processes in pharmacy services. Key Responsibilities:

• Coordinate medication reconciliation for effective care
• Engage in patient care rounds with healthcare teams
• Develop and initiate personalized pharmaceutical care
• Address and troubleshoot medication therapy concerns
• Promote ongoing education for pharmacy staff Requirements:
• Doctor of Pharmacy or accredited residency
• Ontario College Registered Pharmacist
• Relevant hospital pharmacy experience
• Strong teamwork and problem solving abilities
• Demonstrated communication and organizational skills Drive positive health outcomes and deliver exceptional care as part of TBRHSC's dedicated pharmacy team.
