You will be responsible for key performance indicators such as medication reconciliation and developing tailored pharmaceutical care plans. In addition, your skills in collaboration and patient education will be essential for providing compassionate, patient-centered care. Key Responsibilities:

• Lead medication reconciliation and pharmaceutical care plans
• Participate in interprofessional patient care rounds
• Educate patients on medication management
• Resolve drug therapy problems efficiently
• Support mentorship and continuous quality improvement efforts Requirements:
• Doctor of Pharmacy or ACPR
• Registered Pharmacist with Ontario College
• Experience in acute care hospital pharmacy
• Strong critical thinking and communication skills
