Position Summary

To provide case management services for a caseload of clients with severe mental illness who are homeless, at risk of homelessness or transitioning to independent living in the Mississauga/South Etobicoke area. Case Managers work in partnership with clients to develop the skills and supports they need to live successfully in the community. Clients are supported through visits in their own home, in the community or in the office.

The Case Manager works within a Trillium Health Partners community-based case management service for Mississauga residents, aged 18 and over, who have a history of serious and persistent mental illness. They work in partnership with clients to develop the skills and supports needed to live successfully in the community. The successful candidate will meet with clients either in their own home, in the community or in the office to provide recovery-based support.

Responsibilities

Manage a full caseload, providing case management support to assigned individuals

Complete assessment reports outlining individual psychosocial needs and determining eligibility for the program

Engage individuals in the Recovery process through a goal oriented, skills-based approach

Ongoing monitoring of individual’s wellness, assessment of risk and overall progress in one’s Recovery Process

Liaise with existing treatment team including physicians

Qualifications

Professional qualification in a health related discipline

Minimum of 3 years of postgraduate experience in Mental Health; preferably in the community and hospital setting

Through knowledge of the Ontario Mental Health Act and Health Care Consent Act

Excellent communication and organizational skills

Excellent collaboration skills with consumers, families and other healthcare professionals

Exceptional problem-solving and decision-making skills

Demonstrated ability to work independently and as a member of an interdisciplinary team

Knowledge of community resources

Valid driver's license; relevant insurance and use of own vehicle

Ability to speak a second language is an asset

Satisfactory performance and attendance required
