The Case Manager works under the supervision of the Clinical Services Manager. Provides case management services to children in The Children's Home program. This includes on-going treatment planning and treatment interventions.
POSITION QUALIFICATIONS
Education: Bachelor’s Degree in Social Work or related field, with experience working with children, adolescents and families.
Experience: Two years of experience working with children, adolescents and families.
Special Skills: Strong clinical skills with emphasis on special needs children who are emotionally, behaviorally, and/or developmentally delayed or impaired. Strong oral and written communication skills including computer skills. Ability to function as a team player on a multi-disciplinary treatment planning team.
POSITION RESPONSIBILITIES
Establishes and maintains working relationship with community agencies in order to provide the most appropriate assistance to the children in placement.
Demonstrate resource capability through use of other agencies.
Case Management: Provide case management services to clients and their families to ensure the coordination of all therapeutic services provided in conjunction with cottage therapist or mental health provider.
Treatment Planning: Provide leadership to and actively participate as a member of the multi-disciplinary treatment team.
Discharge Planning/Community Support Services: Facilitate and oversee the development, implementation and ongoing coordination of discharge planning efforts.
Consultation: Consults with staff to ensure that each client’s Treatment clinical needs are identified and addressed.
Participates in school conferences regarding the children when appropriate.
Documents all casework and non-clinical services in the child’s clinical record in a timely manner.
Job Type: Full-time
Pay: $23.00 - $26.00 per hour
