Job Description
Position Summary
We are seeking a Social Worker to join our interprofessional allied health team in a new full-time role. In this position, you will provide holistic intake, case management, and system navigation support to refugees and displaced migrants at all stages of their resettlement and acculturation journeys. You will work closely with our primary care, nursing, mental health, and community partner teams to coordinate care, address social determinants of health, and strengthen clients’ safety, wellness, and integration.
Key Responsibilities
- Complete comprehensive psychosocial and social determinants of health assessments, including social histories and risk assessments as needed.
- Provide trauma-informed, culturally responsive case management to newcomers and precariously insured individuals, including safety planning, goal setting, and individualized service plans.
- Assist clients in identifying and prioritizing their goals related to health, housing, income, employment, education, family reunification, and community integration, supporting wellness, empowerment, and autonomy.
- Coordinate care and system navigation across health, social, legal, settlement, and community services; make referrals, advocate for client needs, and provide proactive follow-up to ensure successful connection.
- Work as a core member of the intake and care coordination team, supporting clients with complex mental and physical health issues who require ongoing, wraparound services.
- Collaborate closely with primary care providers (e.g., physicians, nurse practitioners, nurses) to support integrated care and continuity for clients attached to CCRIHC clinics.
- Provide short-term supportive and crisis counselling to individuals and families experiencing trauma, mental health concerns, substance use, and acute psychosocial stressors.
- Apply harm-reduction and anti-oppressive frameworks across all aspects of practice, including education on safer substance use and strategies to reduce health and social harms.
- Build and maintain strong partnerships with settlement, community health, housing, legal, and social service organizations to enhance service coordination and advocacy.
- Maintain up-to-date knowledge of community resources, immigration-related supports, and emerging health and social issues affecting refugee and newcomer communities.
- Accompany clients to appointments (e.g., health, legal, housing, immigration) when needed to support navigation and advocacy.
- Accurately document client interactions, assessments, and care plans; complete required forms and contribute to data collection and statistical reporting.
- Participate in outreach, health promotion, and community engagement activities relevant to refugee and newcomer populations.
- Contribute to the development, implementation, and evaluation of CCRIHC programs focused on social determinants of health and refugee health equity.
- Participate actively in team meetings, case conferences, committees, and special events as required.
- Demonstrate a commitment to a healthy and safe workplace by following all applicable policies, health and safety legislation, and best practices.
Qualifications
- Bachelor of Social Work (BSW) or equivalent degree in social work, social sciences, or a related health discipline; eligibility for registration with the Ontario College of Social Workers and Social Service Workers is preferred.
- Minimum 3–5 years of experience in a community health, settlement, or social service setting, or an equivalent combination of education and experience.
- At least 1–3 years of direct experience working with refugees, asylum seekers, and/or diverse newcomer populations, including individuals impacted by trauma, homelessness, and substance use.
- Demonstrated experience providing case management, system navigation, and short-term supportive or crisis counselling in an interprofessional environment.
- Strong analytical, assessment, and decision-making skills to identify client strengths and needs, resolve issues, and develop appropriate care and service plans.
- Proven ability to work within harm reduction and anti-oppressive frameworks, with cultural humility and sensitivity to the impact of migration, discrimination, and systemic barriers.
- Experience supporting individuals with mental health and/or substance use challenges, including knowledge of community mental health resources.
- Excellent communication skills in English (written and verbal); fluency in a second language commonly spoken by refugee and newcomer communities (e.g., Arabic, Dari, Pashto, Somali, Tamil, Tigrinya, Urdu, etc.) is a strong asset.
