In this full-time, permanent position, your role is to provide holistic, accessible primary care while working alongside an interprofessional team. You will assess, diagnose, and treat diverse health concerns and contribute to outreach initiatives that promote wellness across the community.

Key Responsibilities

  • Provide family-focused primary healthcare services
  • Assess and treat both acute and chronic health issues
  • Develop and execute tailored treatment plans
  • Perform routine immunizations as per protocols

Requirements

  • Master’s degree with Primary Health Care Nurse Practitioner certification
  • Active registration in the Ontario College of Nurses
  • 3–5 years’ experience as an NP in a community setting
  • Current CPR (HCP) certification
  • Excellent interpersonal and communication abilities
