Full-Time Intensive Case Manager for Mental Health and Housing Support

You will serve as a vital resource in the Supportive Addiction & Mental Health Housing program, delivering comprehensive services to individuals facing significant challenges. This position involves independent work, supporting clients via home visits and interactions with community service providers. Evening and weekend work may be required to meet client needs effectively.

Key Responsibilities

• Facilitate client intake and service assessment
• Assist clients with housing move-in logistics
• Create personalized crisis management plans
• Conduct regular home visits for support and assessment
• Work collaboratively with multidisciplinary teams

Requirements

• College diploma or degree in relevant field
• Minimum one year experience in social services
• Solid understanding of case management practices
• Valid driver’s license and reliable vehicle
• Standard First Aid/CPR Level C certification
