Social Worker - Resident Experience Coordinator Role

In this part-time role at Aspira Cedarvale Lodge, your experience in social services will significantly contribute to enhancing resident wellbeing. You will guide their transition into long-term care and facilitate emotional support through counseling and care planning. Your leadership will ensure that families are comforted during critical conversations about care.

Key Responsibilities:

• Manage resident move-ins, transfers, and departures
• Provide counseling services and assess residents’ needs
• Plan and implement support groups as needed
• Assist in advanced care planning conversations
• Mentor social work students during their placements

Requirements

• Registered as a Social Worker per provincial legislation
• At least three years in healthcare or social services
• Familiarity with Long Term Care Act & Regulations
• Excellent communication and organizational skills
• Experience in customer service or sales is beneficial
