Key Responsibilities
- Assess patients’ psychosocial, emotional, and financial needs.
- Provide counseling and emotional support to patients and caregivers.
- Coordinate with doctors, nurses, and healthcare teams regarding patient care plans.
- Assist patients in accessing government schemes, insurance, and welfare services.
- Support discharge planning and rehabilitation arrangements.
- Maintain accurate patient case records and documentation.
- Conduct crisis intervention and grief counseling when required.
- Refer patients to community agencies, NGOs, rehabilitation centers, or support groups.
- Organize awareness programs and patient education activities.
- Participate in multidisciplinary healthcare meetings.
- Ensure confidentiality and ethical standards in patient interactions.
Required Qualification:
- Relevant hospital or healthcare experience is an advantage.
- Strong communication and interpersonal skills
