Join Senior Persons Living Connected as a part-time Occupational Therapist in the GAIN Community Team. Support seniors with personalized interventions and assessments, enhancing their quality of life. This on-site role requires a professional dedicated to senior care.
You will assess and develop interventions for older adults affected by the emotional and physical impacts of aging. Collaborating with an inter-professional team, you will ensure comprehensive care and support tailored to individual needs. Key Responsibilities:
- Conduct Comprehensive Geriatric Assessments (CGA)
- Perform clinical triage for new patient referrals
- Gather and document data effectively for assessments
- Provide clinical care to enhance daily living activities
- Collaborate on care plans with the GAIN team Requirements:
- University Degree in Occupational Therapy
- Registered with the College of Occupational Therapists of Ontario
- 2-3 years of relevant experience
- Knowledge of dementia and geriatric therapy
- Vulnerable Sector Check and driver’s license required Leverage your skills in occupational therapy to make a difference in the lives of seniors at SPLC.
