Clinical Pharmacist- Ruby Hall Clinic Hinjewadi

Roles and Responsibilities:

1. Clinical Responsibilities

• Review and validate medication orders for accuracy and appropriateness
• Monitor patient medication therapy and suggest interventions when required
• Identify and prevent drug interactions, adverse drug reactions (ADRs), and medication errors
• Perform medication reconciliation at the time of admission and discharge
• Assist in dose adjustments based on patient condition (renal/hepatic function)

2. Patient Care

• Counsel patients regarding medication use, dosage, side effects, and adherence
• Educate patients and caregivers on disease management and lifestyle modifications
• Monitor patient response to therapy and report outcomes

3. Collaboration with Healthcare Team

• Participate in ward rounds and clinical discussions with doctors
• Provide evidence-based drug information to physicians and nursing staff
• Support clinical decision-making for optimal patient care

4. Medication Safety & Compliance

• Report and document adverse drug reactions (pharmacovigilance)
• Ensure safe handling of high-risk and emergency medications
• Follow hospital protocols, NABH/JCI standards, and regulatory guidelines

5. Documentation & Reporting

• Maintain accurate patient medication records
• Prepare clinical notes, intervention reports, and audit data
• Assist in developing and updating hospital drug policies

6. Training & Development

• Conduct training sessions for nursing and pharmacy staff
• Stay updated with the latest clinical guidelines and drug information
• Participate in research, audits, and quality improvement initiatives

Preferred Candidate Profile:

• Pharm.D qualification is mandatory
• 3 to 5 years of relevant clinical experience in a hospital setting
• Immediate joiner preferred
• Prior hospital experience is required
• Candidates residing near the Hinjewadi location will be preferred
