<p data-start="579" data-end="667"><strong data-start="579" data-end="597">Working Hours:</strong> Full-time (weekday and weekend availability within a 7-day service)</p>
<p data-start="669" data-end="915">We are currently seeking a dedicated and experienced Physiotherapist to join our client’s team in South-West London. This is a fantastic opportunity to provide short-term home-based rehabilitation for patients recently discharged from hospital.</p>
<p data-start="917" data-end="1384"><strong data-start="917" data-end="941">Key Responsibilities</strong><br data-start="941" data-end="944">You will be responsible for conducting physiotherapy assessments in patients’ own homes, delivering short-term reablement interventions (typically up to 3 days per patient), and developing safe, goal-focused rehabilitation plans. You will work closely with wider support teams to ensure effective handover to longer-term community services, maintain accurate clinical records, and contribute to maintaining high standards of patient care.</p>
<p data-start="1386" data-end="1756"><strong data-start="1386" data-end="1419">Qualifications and Experience</strong><br data-start="1419" data-end="1422">HCPC registration (Health and Care Professions Council)<br data-start="1477" data-end="1480">Degree in Physiotherapy<br data-start="1503" data-end="1506">Experience in community rehabilitation, home-based assessment or reablement – essential<br data-start="1593" data-end="1596">Strong clinical reasoning and communication skills<br data-start="1646" data-end="1649">Ability to work independently and as part of a multidisciplinary team<br data-start="1718" data-end="1721">Previous NHS experience preferred</p>
