Type: Hybrid (3 days in office, 2 days remote)

About The Role

The Clinical Lead is a senior clinical and operational leader responsible for overseeing Nurse Practitioner (NP) services across our retirement home sites. Reporting to the Vice President of Operations, this role bridges strategy and frontline execution - leading our NP team while maintaining active clinical practice through virtual care delivery.

This is a hybrid leadership and clinical role for an experienced NP who wants to shape how primary care is delivered to seniors at scale, while staying close to patients.

Key Responsibilities

  • Clinical Governance and Quality - Maintain and implement national clinical SOPs, ensure compliance with provincial regulations and privacy laws, lead incident reviews and quality improvement initiatives, and audit documentation and clinical performance across sites.
  • NP Team Leadership - Lead a team of over 15 Nurse Practitioners across Canada. Recruit, interview, and hire Nurse Practitioners. Lead structured onboarding from shadowing through ramp-up to independent practice. Provide ongoing clinical, operational, and EMR training. Conduct regular check-ins with NPs to support confidence, performance, and retention, and address performance gaps through structured improvement plans.
  • Operations and Workflow Optimization - Standardize clinic workflows across all sites, optimize scheduling, documentation, and referral processes, and ensure clear role delineation across clinical and administrative teams. Oversee booking accuracy, audit scheduling workflows, and partner with administrative teams to improve clinic flow.
  • Retirement Home Client Liaison - Serve as the primary clinical point of contact for Directors of Care and Executive Directors at partner homes. Address concerns, strengthen communication workflows, and ensure alignment between Perennial's clinical services and partner home expectations.
  • Strategic Support to the VP of Operations - Translate strategy into actionable workflows, provide operational insights and recommendations, and support growth and scaling initiatives as Perennial expands across provinces.
  • Systems and Resource Coordination - Triage operational and clinical system issues (e.g., Bloomic EMR), route inquiries to the appropriate teams, and escalate recurring issues. Monitor clinical and operational supply availability and ensure care is never disrupted by inventory gaps.
  • On-Site Leadership and Travel - Travel to sites for new clinic launches, NP onboarding, performance optimization, and relationship building with partner homes.
  • Virtual Clinical Care Delivery - Maintain active clinical practice through virtual appointments across Ontario, Saskatchewan, Alberta, and Manitoba. Provide primary, episodic, and urgent care within scope. Support coverage gaps, model best practices for virtual workflows, documentation, and patient communication, and complete documentation within 24 hours of each encounter. This dual mandate ensures the Clinical Lead remains clinically credible, operationally informed, and responsive to system needs. Virtual clinical care delivery is about 10% of your total time in this role.

Qualifications

Required

  • Nurse Practitioner licensed in Ontario, in good standing
  • Willingness and ability to obtain NP licensure in Saskatchewan, Alberta, and Manitoba
  • 2+ years of clinical experience as an NP
  • Leadership, supervisory, or team lead experience
  • Strong understanding of primary care workflows and documentation standards

Nice to Have

  • NP licensure in additional provinces like Saskatchewan, Alberta, and Manitoba is considered an asset
  • Experience in geriatrics, retirement homes, or long-term care
  • Multi-site or program-level leadership experience
  • Experience with EMRs
  • Experience delivering virtual care

Job Type: Full-time

Pay: $130,000.00-$150,000.00 per year
