Job Description

  • Review and evaluate physician medication orders for accuracy, dosage, and drug interactions.
  • Participate in clinical ward rounds with doctors and healthcare teams.
  • Monitor patient response to drug therapy and recommend appropriate changes.
  • Identify and report Adverse Drug Reactions (ADR) and medication errors.
  • Provide patient counseling on medication use, side effects, and adherence.
  • Perform prescription audits for inpatient and outpatient departments.
  • Assist in development and maintenance of hospital drug formulary and antibiotic policies.
  • Conduct medication reconciliation during patient admission and discharge.
  • Train nursing staff and junior pharmacists on medication safety.
  • Maintain documentation related to clinical pharmacy interventions and patient care.
  • Candidate should be flexible and willing to take up additional responsibilities as assigned by management when required.

Qualifications

  • Pharm.D / M.Pharm
  • Registered Pharmacist with State Pharmacy Council.

Experience

  • 2–4 years for Pharm.D /M.Pharm graduates Hospital clinical pharmacy experience preferred

Pay: ₹20,000.00 - ₹35,000.00 per month
