Job Description
- Review and evaluate physician medication orders for accuracy, dosage, and drug interactions.
- Participate in clinical ward rounds with doctors and healthcare teams.
- Monitor patient response to drug therapy and recommend appropriate changes.
- Identify and report Adverse Drug Reactions (ADR) and medication errors.
- Provide patient counseling on medication use, side effects, and adherence.
- Perform prescription audits for inpatient and outpatient departments.
- Assist in development and maintenance of hospital drug formulary and antibiotic policies.
- Conduct medication reconciliation during patient admission and discharge.
- Train nursing staff and junior pharmacists on medication safety.
- Maintain documentation related to clinical pharmacy interventions and patient care.
- Candidate should be flexible and willing to take up additional responsibilities as assigned by management when required.
Qualifications
- Pharm.D / M.Pharm
- Registered Pharmacist with State Pharmacy Council.
Experience
- 2–4 years for Pharm.D /M.Pharm graduates Hospital clinical pharmacy experience preferred
Pay: ₹20,000.00 - ₹35,000.00 per month
