POSITION: Clinical Pharmacist – 1.0 FTE Permanent
REPORTING TO: Director, Programs & Services or their delegate
UNION AFFILIATION: This position is part of the Ontario Nurses’ Association (ONA) bargaining unit and will be subject to the terms and conditions outlined in a collective agreement. Union dues will be applicable as per ONA guidelines.
POSITION SUMMARY:
North York Family Health Team Inc. (NYFHT) provides comprehensive, team-based primary care to approximately 95,000 patients. The Clinical Pharmacist is responsible for providing patient-centered pharmaceutical care, medication assessments, pharmacotherapy consultation, and medication education as part of the interdisciplinary primary care team.
MAIN DUTIES AND RESPONSIBILITIES:
- Conduct comprehensive medication assessments to identify, prevent, and resolve drug therapy problems
- Provide pharmacotherapy consultation to physicians and interdisciplinary team members
- Develop and implement medication management plans in collaboration with patients and the care team
- Monitor pharmacotherapy outcomes and support medication, safety and adherence
- Provide patient education regarding medications, treatment options, and self-management strategies
- Delivering evidence-based drug information to patients and health care providers
- Participate in multidisciplinary group education programs
- Contribute to chronic disease management initiatives, disease prevention, and health promotion activities
- Participate in quality improvement initiatives and program evaluation
- Provide clinical teaching to residents, students, and other health professionals where appropriate
- Participate in interdisciplinary case conferences and care planning discussions
- Maintain accurate and timely clinical documentation within the electronic medical record (EMR)
- Participating in committees, community partnerships, and program development initiatives
- Perform other duties as assigned
JOB ENVIRONMENT: Clinical setting within a community-based primary care medical office environment.
QUALIFICATIONS AND EXPERIENCE:
- Bachelor of Science in Pharmacy or PharmD from a recognized institution
- Registration in good standing with the Ontario College of Pharmacists (Part A license)
- Minimum of 3 years relevant clinical pharmacy experience
- Demonstrated knowledge of pharmacotherapy and medication management in chronic disease
- Experience providing patient-centered care and medication counseling
- Experience working within an interdisciplinary team environment
- Strong knowledge of evidence-based pharmacotherapy and medication safety practices
- Excellent communication, interpersonal, teaching, and organizational skills
- Demonstrated critical thinking and problem-solving abilities
- Ability to work independently and collaboratively in a multidisciplinary team
- Experience using electronic medical record systems (Accuro/PSS) considered an asset
- Experience in a Family Health Team or primary care setting considered an asset
- Pharmacy practice residency or additional clinical certification considered an asset
- Occasional flexibility in scheduling may be required to meet operational needs.
COMPENSATION & BENEFITS:
Compensation is in accordance with the Collective Agreement.
