Job Summary
This position is part of an inter-disciplinary team responsible for the monitoring and licensing of Personal Care Homes. The primary responsibilities of this position will be case management and to ensure compliance to standards as outlined in the Personal Care Home Program Operational Standards 2007 and the Personal Care Home Legislation 2001. This will include monitoring activities and completing related reports for assigned Personal Care Homes. In this role the Social Worker will participate in team meetings, licensing meetings and investigation of complaints and must have the ability to work independently with limited supervision. The Social Worker will document in CRMS and complete monthly statistical reports as required. The successful candidate will provide in-service/education to PCH operators and staff on topics including, but not limited to, documentation, incident reporting and the Adult Protection Act, as well as developing presentations with the interdisciplinary team. The Social Worker is also responsible for adult protection referrals and provides case management services for residents of personal care homes including assessments, reassessment, transfer coordination, access to and authorization of eligible services. This position is expected to provide field instruction for Social Work students.
This position reports to the Regional Manager of the Personal Care Home Program and maintains accountability to the Social Work Professional Practice Consultant for professional integrity and practice issues.

Job Qualifications
Graduation with an accredited Bachelor of Social Work Degree is required.
Current and continuing registration with the Newfoundland and Labrador Association of Social Workers is required.
Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act, Medical Assistance in Dying, and the Children, Youth and Families Act.
Knowledge of community resources, systems and programs.
Knowledge of crisis intervention and supportive counseling.
Knowledge of case management.
Demonstrated ability to direct, supervise and mentor students.
Demonstrated clinical skills in assessment and interventions with individuals, families and groups.
Demonstrated analytical, critical thinking, problem solving, decision-making and organizational skills.
Demonstrated strong written and oral communication skills as well as the ability to deliver/facilitate effective presentations.
Demonstrated ability to work effectively within an interprofessional environment, and establish and maintain effective working relationships.
Demonstrated ability to work independently.
Conditions of employment include the use of a private vehicle.
A satisfactory record of work performance and attendance is required.
