Clinical Care Facilitator Nurse Practitioner Role

Join the East Toronto Health Partners as a Clinical Care Facilitator Nurse Practitioner. This role focuses on advanced clinical care for high-risk ICU recovery patients. In this position, you will be a key member of the interdisciplinary VENT+ Pathway team, focusing on patients with complex medical needs post-ICU.

You will lead ongoing assessments, create tailored care plans, and facilitate communication among caregivers and healthcare providers. Empathy, strong clinical judgment, and skills in navigating fragmented care systems are crucial for effectively supporting patients through challenging recovery stages. Key Responsibilities:

• Assess and manage complex post-ICU patient needs
• Build relationships for improved post-discharge transitions
• Conduct thorough assessments and continuous monitoring
• Coordinate care with interdisciplinary teams
• Develop and implement individualized recovery plans Requirements:
• Licensed Nurse Practitioner with adult health specialty
• At least 3 years in critical or respiratory care
• Excellent interpersonal and communication abilities
• Proven experience in collaborative healthcare settings
