Job Description

Become a Case Manager for elderly support services and drive impactful change. Facilitate care coordination for seniors facing serious mental illness and fosters community linkages. In this full-time position, you will create compassionate care plans while addressing the varying needs of clients. Responsibilities include conducting assessments, crisis management, and collaborating with family and healthcare professionals to enhance well-being and independence.   • Degree in social services or a related field
