ED Case Manager

LifeBridge Health seeks an ED Case Manager Social Worker to coordinate care for patients in our emergency department. In this role, you will assess psychosocial needs, develop discharge plans, arrange community services, and collaborate with physicians, nurses, and community partners to ensure safe, efficient patient transitions. You will advocate for patients and families, address barriers to care, and support crisis intervention.
