Patient Care Coordinator

SUMMARY: Collaborates with the patient and their families, the patient’s physicians, and all care providers in coordinating the care of the patient, utilizing skills of advocacy, communication and education. Identifies, facilitates and coordinates the appropriate services, resources, providers and facilities throughout the continuum of care.

REQUIREMENTS

• Minimum Experience: 6 months experience preferred
• Required License/ Certifications: Current CA RN License, BLS and MAB Certificate (90 days upon hire) required

AGE SPECIFIC: Patient care employees must be able to demonstrate the knowledge and skills necessary to provide care appropriate to patients of all age groups–must demonstrate. knowledge of the principles of growth and development through the life span, possess the ability to assess data reflective of the patient’s status, and interpret the appropriate information needed to identify each patient’s needs in relation to age and to provide the care needed as described in the department’s policies and procedures.
