Job Description
Act as a Patient Liaison and Case Manager, collaborating with healthcare providers to improve patient support services. Drive relationship-building and communication to resolve client issues effectively. In this role, you will oversee the maintenance of patients enrolled in programs while attending educational events. Your experience in nursing and case management will facilitate training sessions for both physicians and clinic staff. This position involves working closely with multiple stakeholders to ensure continuous improvement in service quality. Key Responsibilities:
- Attend educational events to report on new activities
- Establish relationships with physicians and office staff
- Provide in-service presentations on program updates
- Liaise with the Patient Assistance Program for feedback
- Maintain comprehensive documentation of patient interactions Requirements:
- RN or RPN registration required
- Three years of nursing experience in clinical settings
- Ability to travel within the assigned territory
- Knowledge of reimbursement processes and systems
