Salary or Pay Band: Pay Band Social Worker - BSW $35.137 to $42.966

Job Description: The social worker is responsible for assisting residents and families in adjusting to life in a long term care facility. As well, they will provide social assessment and casework services as part of an interdisciplinary team. The social worker requires strong team interaction skills, the ability to maintain long term professional relationships with residents, and an ability to function with clear and professional boundaries within a residential and institutional home setting.

The incumbent will share social work responsibilities in the department including coverage for holidays and sick leave. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Education

  • Bachelor of Social Work (BSW)

Licenses

  • Registered Social Worker (RSW)

Other Education and Training

  • Group work training is an asset

Competencies

  • Case management skills
  • Basic - Communication skills

Knowledge and Abilities

  • Ability to work independently
  • Ability to work as a member of a multi-disciplinary team
  • Abilty to advocate on behalf of clients
  • Knowledge of Psycho-social assessment in a long term care setting
  • Knowledge in discharge planning
  • Ability to work and communicate in a cross cultural setting
  • Knowledge in the SHA, community resources and applicable programming
  • Knowledge of discharge planning in a long term care setting

Other Information

  • Experience working in health care, preferable long term care, is preferred
  • Experience in community development training is an asset
