Social Worker- Full-time

Greenwood House Home for the Jewish Aged, a highly respected Five Star 137 Bed Skilled-Nursing and Rehab Facility in Ewing, NJ that has served the community for over 70 years is seeking a full-time geriatric social worker with solid experience in both long-term and sub-acute care. Master’s degree in social work and New Jersey LSW or LCSW required. This position will primarily support our long-term care residents, helping to enhance their quality of life and ensure needs are met.

Key responsibilities include:

• Evaluate existing, new and prospective patients based on their needs, desires and limitations
• Maintain constant communication with residents while addressing their concerns and goals and helping them keep positive attitudes
• Coordinate with PT and OT departments to assist in helping residents feel comfortable and confident
• Ensure compliance with regulatory bodies and in-house clinical guidelines
• Build rapport with residents/patients and their families and relay all concerns to supervisory case manager
• Develop care plans for residents and provide therapeutic support as needed

Candidate qualifications:

• Valid license necessary
• 2-3 years clinical social work experience
• Valid and current LSW or LCSW license
• Ability to make psychosocial assessments and develop and implement viable care plans
• Excellent listening and communication skills
