Job Description

The Role

As a Nurse Practitioner (NP) in the Primary Health Care department, you will provide comprehensive primary care services, including health promotion, disease prevention, diagnosis, and treatment of acute and chronic conditions. Working collaboratively with physicians, nurses, and other healthcare providers, you will ensure that patients receive holistic, patient-centered care.

Responsibilities

• Perform comprehensive health assessments, including physical exams and diagnostic tests, to diagnose and manage a wide range of health conditions.
• Develop and implement treatment plans for acute and chronic illnesses, prescribing medications as needed.
• Provide health education and counseling to patients on lifestyle, preventive care, and chronic disease management.
• Collaborate with interdisciplinary teams to ensure continuity of care and effective care planning.
• Monitor and evaluate patient progress, adjusting treatment plans as necessary to achieve optimal health outcomes.
• Maintain accurate and detailed patient records, documenting assessments, treatment plans, and follow-ups.

Requirements

• Completion of a Nurse Practitioner program and relevant training.
• 2 years of experience in a primary care or clinical setting is preferred
• Class 5 Drivers License is required.
• Will also need to complete PART, TLR, WHMIS, WAVE during onboarding if not already completed.
• RN, AAP, ITLS, BLS, PALS, ACLS is Required.

Compensation

$90-93/hr plus 4% - Employee

$93-95/hr for - IC .
