BLS, LCSWSkills : Outpatient Clinic, Physician offices/ Private Practice, Psychiatric Hospital, Biopsychosocial assessment, Assists patients with navigating the Department of Children and Families (DCF) process, Collaborative care, Coordinate services to home care and equipment following discharge, Documents pertinent information to facilitate care coordination, Participates in care conference activities, Refers to rehab and long term care facilities, Refers to other disciplines/resources, Individual counseling, Marriage counselling, Parental counseling, AFDC (Aid to Families with Dependent Children), Food stamps, Medicaid, Medicare, Medication Assistance, TANF (Temporary Aid for Needy Families), Adult protective services, Child protective services, Emergency detention, Guardianship, Power of Attorney, Adoption / Foster Care, Drug/alcohol use and treatment options, 5150/ Involuntary holds, Participate in development of treatment plan
