This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.</strong></span></p>

<p><span style="font-size: 10pt;"><strong>KEY RESPONSIBILITIES</strong></span></p>
<ul>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Coordinate care for long-term care residents using Ennoble Care’s EMR</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Collaborate closely with Ennoble Care providers, within the long-term care setting</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Serve as a key liaison between providers, facility staff, residents, and families</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Accurately document patient interactions and care coordination activities</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Follow established workflows, documentation standards, and escalation protocols</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Contribute positively to team collaboration and strong facility partnerships</span></li>

<p><span style="font-size: 10pt;"><strong>WHAT 'EMBEDDED' MEANS AT ENNOBLE CARE</strong></span></p>
<p><span style="font-size: 10pt;">This is not a remote or call-center role.</span></p>
<p><span style="font-size: 10pt;">As an Embedded Care Coordinator, you are physically on-site in one or more partnered long-term care facilities, working alongside providers and facility staff. You build consistent relationships with residents and families and become a trusted, familiar presence within the facility -- supporting care the way a traditional family practice would, but in the long-term care setting. To support a dedicated group of patients. The work is very much family-practice focused, including a strong emphasis on remote patient monitoring (RPM) and proactive care coordination.</span></p>

<p><span style="font-size: 10pt;"><strong>REQUIREMENTS</strong></span></p>
<ul>
<li><span style="font-size: 10pt;">Legally authorized to work in the United States of America</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Active, unrestricted clinical certification or licensure, such as:</span></li>
<ul>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Patient Care Technician (PCT)</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Certified Nursing Assistant (CNA)</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Medical Assistant (MA) / Certified Medical Assistant (CMA)</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Licensed Practical Nurse (LPN)</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Registered Nurse (RN)</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Or comparable credential</span></li>
</ul>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Strong working knowledge of healthcare terminology and care coordination principles</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Proficiency with Microsoft 365, including Outlook, Word, and Excel</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Reston, Virginia area</span></li>
<ul>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">No weekends</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">No on-call</span></li>
<li style="font-size: 10pt;"><span style="font-size: 10pt;">Daytime hours</span></li>
