We seek individuals who are driven to make a difference and embody our motto, “To Care is an Honor.” Join Ennoble Care today!</div>Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Reston, Virginia area. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support. We support internal advancement and long-term career growth based on performance, organizational needs, and role availability. <hr><hr> KEY RESPONSIBILITIES <ul> <li style="font-size: 10pt;">Coordinate care for long-term care residents using Ennoble Care’s EMR</li> <li style="font-size: 10pt;">Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions</li> <li style="font-size: 10pt;">Collaborate closely with Ennoble Care providers, within the long-term care setting</li> <li style="font-size: 10pt;">Serve as a key liaison between providers, facility staff, residents, and families</li> <li style="font-size: 10pt;">Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner</li> <li style="font-size: 10pt;">Accurately document patient interactions and care coordination activities</li> <li style="font-size: 10pt;">Follow established workflows, documentation standards, and escalation protocols</li> <li style="font-size: 10pt;">Contribute positively to team collaboration and strong facility partnerships</li> </ul> <hr> WHAT 'EMBEDDED' MEANS AT ENNOBLE CARE This is not a remote or call-center role. As an Embedded Care Coordinator, you are physically on-site in one or more partnered long-term care facilities, working alongside providers and facility staff. You build consistent relationships with residents and families and become a trusted, familiar presence within the facility -- supporting care the way a traditional family practice would, but in the long-term care setting. To support a dedicated group of patients.
