This role is an integral, in-office member of our clinical care team, working alongside fellow Care Coordinators and collaborating with Ennoble Care providers within a family-practice-style model of care.</strong></span></p>
<p><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><strong>While not patient-facing, the position plays a critical role in supporting patient-centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow-up across care settings.</strong></span></p>

<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Coordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) system</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Support Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordination</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Serve as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination priorities</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Assist with care transitions, referrals, and coordination with external partners, as needed</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Collaborate closely with fellow Care Coordinators and internal care team members within an office-based environment</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Maintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support timely decision-making and efficient clinical operations</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Contribute positively to team collaboration, workflow efficiency, and day-to-day office operations</span></li>

<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Medical Assistant (MA) / Certified Medical Assistant (CMA)</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Licensed Practical Nurse (LPN)</span></li>

<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and internal systems</span></li>
<li style="font-family: helvetica, arial, sans-serif; font-size: 10pt;"><span style="font-family: helvetica, arial, sans-serif; font-size: 10pt;">Cultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presence</span></li>
