Job Description
Full-Time or Part-Time Clinical Pharmacist Up to 37.5 Hours per Week (0.5-1 FTE) Permanent The Clinical Pharmacist is an integral member of the DAFHT interdisciplinary team, supporting the organization’s vision and strategic objectives through collaborative primary care and improved medication management.
DUTIES AND RESPONSIBILITIES - Collaborate with physicians and the interdisciplinary team to promote optimal, patient centered care - Conduct comprehensive medication assessments to identify, prevent and resolve medication therapy problems - Develop individualized care plans and monitor therapeutic outcomes - Provide accurate, timely, and evidence-based drug information to physicians and the interdisciplinary team to support safe and effective prescribing decisions - Conduct Medication Reconciliation at transitions of care to ensure medication regimens are accurate and safe - Develop and deliver Chronic Disease Management Programs for osteoporosis, chronic pain management, dementia, insomnia, hypertension, mental health etc.
- Develop patient education workshops, patient materials, prescribing tools for physicians, education session for prescribers and the interdisciplinary, contribute to DAFHT monthly newsletter etc - Provide patient education regarding medications, disease and self-management strategies EDUCATION/EXPERIENCE - Bachelor of Science in Pharmacy (B.Sc.Phm), PharmD or equivalent is required - Registration and in good standing with the Ontario College of Pharmacists (Part A) is required - Hospital or Primary Care Residency, or equivalent clinical experience (minimum of 2 years) is required - Experience working in an interdisciplinary team environment is required - Additional training in pharmacotherapy i.e.
Certified Geriatric
Pharmacist, Opioid Dependence, etc. an asset - Experience with electronic medical records (EMR) systems (i.e. Telus PS Suites) an asset SKILLS/ABILITIES - Motivated and self-directed - Ability to work independently within a collaborative interprofessional team environment - Excellent written and verbal communication skills - Excellent interpersonal and collaboration skills - Ability to prioritize, manage time effectively and be flexible in an active work environment - Strong ability to critically appraise and synthesize evidence from clinical literature and guidelines to develop clear, practical, and patient-centred recommendations within primary care.
