Contract Care Coordinator Assessor, LSW or RN (Part-time or Full-time)

Flexible timeframe of work within regular business hours.&nbsp;</p> <p><strong>Location</strong>: Approximately 70% work from home &amp; 30% in-person travel to meet with members in Hennepin County, MN (Mainly throughout Minneapolis and St. Paul, MN with mileage is reimbursed per company and IRS guidelines).</p> <p><strong>Pay Range</strong>: Paid per assessment - Averages $35 per hour (hourly amount within range is based on experience).&nbsp; &nbsp;</p> <p><strong>Duration of Assignment</strong>: ASAP thru September/October 2026 with the potential of Temp to Full-time opportunities available.</p> <p><strong>Benefits</strong>: Not benefits eligible as a contract employee.</p> <p><span style="text-decoration: underline;"><strong>Responsibilities:</strong></span>&nbsp;</p> <p>As a&nbsp;<strong>Contract Care Coordinator Assessor</strong>, you will complete assessments within your assigned geographic area - <strong>Hennepin County, MN</strong>.</p> <p>This includes:</p> <ul> <li>Contacting new enrollees to complete the initial assessment.
