Job Summary

The Social Worker is an integrated member of the Austin Centre Virtual Behavior Medicine program (VBM) & the Sam & Ida Ross Memory Clinic. Working as a member of the interprofessional team, the Social Worker will provide focused assessment, treatment plans and interventions for outpatients referred to these clinics. This role will also provide consultation support for specific cases referred from specialists within the Austin Centre Movement Disorders, Multiple Sclerosis & General Geriatric Neurology clinics. The Social Worker will refer to other Home and Community Care Support services as the individual client clinical need necessitates. The Social Worker in this role is an expert at supporting patients and families living with cognitive impairment, dementia and complex neurological disorders and proactively supports the clinical team in meeting patient and family social support and counselling needs. The Social Worker leads through practice excellence and functions as an expert working to the full scope of their professional role. This position reports directly to the Clinical Manager of the Austin Centre & indirectly to the Manager & Interprofessional Practice Chief for Social Work.

Key Responsibilities:

  • Provide full range of clinical social work services (psychosocial assessments, case management, clinical triage & intake, individual and family counseling and education) to clinic patients and their families
  • Provide information and make referrals on a range of support services offered at Baycrest, within the Community & Province, offered on–line and in-person e.g. (Learning the Ropes program, Alzheimer Society, Respite Support, Day Programs, Holocaust Survivor Supports, 211 Ontario)
  • Support patients and family with future planning (especially helpful when client receives a diagnosis)
  • Co-ordinate family meetings and support family units when conflicts arise (support family in conflicting perspectives and decision making for a loved one)
  • Responsible for referrals to HCCSS (OT Home Safety Assessment, PSW, LTC applications etc.)
  • Act as lead to determine POA status, obtain copies of POA documents and manage POA concerns on behalf of the patient as needed to support care provision by the team
  • Maintain accurate, up to date clinical documentation and proactively provide direct patient status updates to physicians and other members of the clinical team as appropriate

Key Qualifications:

  • Masters of Social Work degree required
  • Current and valid registration with the Ontario College of Social Workers and Social Service Workers (‘OCSWSSW’) in good standing required
  • Experience with identifying and using a range of clinical assessment tools eg .Suicide Risk Assessment, Depression Scales
  • Experience working with people (Adults & Seniors) living with dementia, cognitive impairment, mental health issues, and families/caregivers across clinical settings and able to provide the spectrum of supports (i.e. prevention, crisis, self-management, treatment etc.) and a comprehensive understanding of the diverse needs of patients & caregivers
  • Expert knowledge of Ontario LTC, Hospital & Community care environments, pertinent Health Services legislation (i.e. Privacy, LTC Act, Hospital Act, MAID), Clinical Ethics, Ontario Senior Support Services
  • Excellent interpersonal communication, solution oriented & problem-solving skills
  • Proficient computer skills & recent experience with Outlook, Meditech, Word, Excel, Resource Matching and Referral (RM&R) MDS required and OTN/ZOOM Virtual Care delivery preferred
