Wage Rate:
The starting wage rate is $36.06/hour.

Position Overview
Coordinates and oversees all aspects of resident-focused delivery of care.
Provide direct supervision, leadership and education to other members of the health care team. Acts as a resource and advocate for residents, families and staff.
Promote the values, goals and philosophy of the Home.
Promotes a safe and secure environment for all residents.
Essential Duties
Assessment of Wounds
- Assess wounds as per the Skin Care and Wound Management Protocol
- Ensure that the skin risk assessment is completed on admission including head
to toe assessment
- Assess/educate residents with actual skin breakdown weekly in
WoundRounds and upload into PCC.
- Assess residents that are re-admitted back from the hospital or returned from a
leave of absence >48 hours with skin breakdown.
Coordinate with the other health care team
- Coordinate with the Assistant Director of Nursing if a referral of a
resident to an Enterostomal Therapist Consultant is required.
- Inform the Assistant Director of Care to order/rent a therapeutic surface as
necessary.
- Provide the dietician a list of residents with actual skin breakdown
monthly and provide access to WoundRounds.
- Coordinate with the physician and the ET consultant any resident(s)
that needs to be referred to a Plastic Surgeon.
Intervention(s)
- Assist the Registered Staff with the dressing of complex wounds as
per MD orders.
Quality Improvement/Documentation
- Ensure that there is a weekly entry in PCC progress notes through
WoundRounds of the resident’s wound status when resident(s) has altered skin
integrity.
- Ensure that the treatment orders on the Physician’s order sheets,
eMAR, and WoundRounds are updated and accurate.
- Check that Care Plans are up to date.
- Analyze data collected of wound prevalence in WoundRounds
- Ensures that quarterly head to toe assessments are done
Quality/Risk Management
- Required to understand the nature and meaning of quality indicators used by the
Home and to understand which indicators are publicly reported.
- Required to participate in the improvement of the indicators and achieve
satisfactory results.
Education
- Coordinate with the Director of Nursing/ Assistant Director of
Nursing or the Staff Development Coordinator on education on skin
and wound management.
Responsibilities to Health and Safety
- Is aware of employee’s responsibilities and follows all Health and Safety policies
and procedures.
- Works safely to reduce the risk of injury to self, co-workers and residents
- Is alert to and properly reports all actual or potential hazardous situations to
immediate supervisor.
- Is aware of WHMIS regulated products and takes safety precautions
accordingly
- Does not operate or use faulty equipment
- Promptly reports personal injury to immediate supervisor and seeks first aid as
needed
- Renders first aid as required
- Has a good understanding of the “Worker’s Responsibilities” under
the Occupational Health and Safety Act.
- Is aware of how and when to use personal protective equipment (PPE).
- Attend all mandatory health and safety training sessions
- Report hazards that you are aware of to your Supervisor and the JHSC
- Understand and follow all of the Home’s health and safety policies and
procedures
General
- Other duties as assigned
Qualifications
- Current Certificate of Competence with the College of Nurses of Ontario as Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
- Basic CPR
- Experience in long term care setting an asset
- Ability to work with and relate to interdisciplinary team members
- Ability to work from the heart and make others smile
- Well-developed written and oral communication skills
- Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one (1) year of being hired.
- Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, must be able to provide three supervisory references.
