Contribute to the wellness of 2SLGBTQ youth at Sherbourne as a full-time Case Manager (Youth Resource Worker). Help support young adults in achieving their goals through comprehensive case management. As a key member of the Community Programs team, you’ll perform intake assessments and provide tailored support to 2SLGBTQ youth aged 14-29.
Your role will involve collaborating with various partners, developing Smart plans, and ensuring access to essential health services. Strong interpersonal skills and crisis management abilities will be vital. Key Responsibilities:
- Conduct intake assessments and identify goals for service users
- Facilitate communication with service users and partners
- Provide brief counselling and urgent support as needed
- Organize workshops and outreach for group activities
- Maintain accurate records in the electronic medical system Requirements:
- Post-secondary education in social work
- Minimum 3 years experience with marginalized youth
- Knowledge of Electronic Medical Recording systems
- Strong problem-solving and professional communication skills
