Licensed Practical Nurse (LPN) – In-Home Assessments

Position Overview

We are seeking Licensed Practical Nurses (LPNs) to complete in-home patient assessments on a recurring basis. Assessments are conducted approximately every 80–90 days per patient, offering consistent, predictable work without shift requirements.

This role is ideal for nurses looking for flexible, independent work with straightforward visits and clear expectations.

Key Responsibilities

• Complete in-home patient assessments according to program guidelines
• Accurately document findings and submit assessments on time
• Assess patient functional abilities, medical needs, and home environment
• Communicate assessment outcomes with the care coordination team as needed
• Maintain professionalism, compliance, and patient confidentiality

Compensation Structure

• $40 per assessment when visits are scheduled by the agency
• $60 per assessment when the nurse schedules directly with the patient
• No minimum or maximum visit requirements
• Assessments recur every 80–90 days, allowing for repeat patients

Qualifications

• Active LPN license in good standing
• At least 1 year of nursing experience preferred
• Reliable transportation and ability to travel locally
• Strong documentation and organizational skills
• Comfortable working independently in the field
• Basic computer skills for electronic documentation

Preferred (Not Required)

• Home health or in-home assessment experience
• Familiarity with Medicaid or waiver programs
• Experience completing patient evaluations
