Job Summary

Nuvella Haven Homecare Staffing Canada is seeking a highly organized and compassionate Transition Coordinator to support students with developmental disabilities as they transition into, through, and out of Inclusive Postsecondary Education (IPSE) programs.

This role is essential in ensuring students experience smooth and supportive transitions between:

• High school and post-secondary education
• Service providers
• Academic terms
• Support teams
• Employment pathways

The successful candidate will coordinate communication between:

Education (IPSE) programs by helping them:

• Students
• Families
• Post-secondary institutions
• Faculty
• Support staff
• Employers, and
• Community agencies

This position requires strong case coordination skills, excellent communication, and the ability to manage multiple stakeholders and timelines simultaneously.

Responsibilities

Student Intake & Onboarding

• Coordinate intake and orientation processes for new students
• Support onboarding into post-secondary environments
• Ensure students understand available supports and resources

Transition Planning

• Develop individualized transition and continuity plans
• Coordinate transitions from high school, previous service providers, community programs, and other educational settings
• Support transitions between academic terms and semesters

Stakeholder Coordination

• Coordinate communication between students, families, faculty, disability service offices, support staff, and employers
• Participate in multidisciplinary planning meetings

Continuity & Risk Management

• Ensure continuity of supports during staffing or service changes
• Identify risks that may impact student success
• Implement mitigation strategies to minimize disruptions

Documentation & Reporting

• Maintain accurate student records and case notes
• Monitor transition outcomes and timelines
• Support reporting requirements and program documentation

Requirements

• Diploma or degree in - Vocational Rehabilitation, Human Services, Psychology, Social Work, Disability Studies or related field
• Minimum 3 years experience in - Disability Services, Transition Planning, Case Coordination, and Educational Support services
• Strong communication and organizational skills
• Experience coordinating multiple stakeholders
• Clear Criminal Record Check & Vulnerable Sector Check
