Become a part-time Social Worker 1 with Horizon in the Upper River Valley area. Engage with diverse communities to enhance health and wellness through collaborative care and education.
This permanent part-time position requires a registered Social Worker dedicated to assessing individual and group needs. You will employ a population health approach through supportive counseling, health promotion, and advocacy. Work frequently with multidisciplinary teams, and leverage partnerships with various organizations to address social determinants of health.
